He was asked to leave the Velvet Underground over creative differences with Lou Reed.
He abused cocaine, leading to often erratic behavior.
He chopped off the head of a chicken in concert, prompting his band to walk offstage.
He wore a goalie mask onstage years before 'Friday the 13th.'

He pushed the Velvet Underground into more experimental music on their first two albums.
