He tortured animals as a boy.
He beat up people and raped women for fun.
He married about seven or eight times.
When the famous St. Basil's Cathedral was built, he had the architects' eyes put out so they could never design another building to rival its beauty.
He formed a brutal secret police, the Oprichniki, to destroy his enemies.
He had the city of Novgorod destroyed and its people barbarically slaughtered.
He killed his eldest son in a violent outburst.

Why he might not be annoying:

He was neglected and abused as a child.
His first marriage was a very happy one (it is said that his first wife's death pushed him over the edge).
He reformed Russia's legal system.
He expanded Russia's borders.
He was the first Russian ruler to called tzar.
