When he and his brother Geta served as co-emperors, they divided the royal palace in two with separate entrances (just like a bad sitcom episode).
He ordered the assassinations of his brother, his wife and his father-in-law.
After Geta's assassination, he ordered all images of Geta destroyed.
After the citizens of Alexandria wrote satires about him, he had hundreds of the city's youth slaughtered.
He was convinced he was the reincarnation of Alexander the Great.

Why he might not be annoying:

He granted Roman citizenship to all free men in the empire.
He started one of Rome's last great building projects, the Baths of Caracalla.
He was assassinated while going to the bathroom.
